This patch updates the Initial setup section from running-your-own
document. Other sections will be updated in separate patches to
make the reviwes short.

The nova project core reviewers have had this enabled for them for
several months without incident, and it's already something anyone
can do locally and push to a change. Also it's convenient when
people are editing commit messages via the WebUI and need to rebase
dependent changes after doing so.

* Use the transfer ownership function in github.
* Move the github step to within the gerrit downtime.
* Indicate that github and git0X steps should be run during the
lucene reindexing (for time efficiency).

This change mainly removes the reference to Smokestack, since the testing
shown at this link is almost a year old. The requirement for wiki records
for accounts was also moved to the Requirements section.

We ended up going with <project>-ci as the name for these third party

We are moving to self service third party CI accounts. Document how that
affects third party testers in this change. Basic TL;DR is using
launchpad openid accounts so that accounts can be self managed. Then put
them in project specific voting tester groups when those accounts are
ready to vote.
