Currently, the proxy troubleshooting guide does not include the IP
address of the Kubernetes API in the defined environment or Ansible
playbook variables. This causes deployments to fail when requests are
routed through proxy servers. This change adds a proxy configuration
section to the beginning of the installation guide and includes the
process of adding the Kubernetes API IP address to the environment and
Ansible playbook variables. It also removes the unecessary sections from
the troubleshooting guide and upgrades the note about resolv.conf being
overwritten to a warning.

Updates references to Launchpad to now point to Storyboard, since OSH has
migrated to that platform. "Report a bug" type references are updated to
point straight to the OSH project proper in Storyboard, while general backlog
type references are updated to point to the OSH Project Group.

trying to use OpenStack-Helm behind a corporate firewall and
require the need of a proxy to reach the internet.

This PS updates the output from the troubleshooting guide to reflect
the expected output and commands for ceph luminious.

This PS deletes superfluous documentation and empty files from
the troubleshooting section, and also updates some of the docs
regarding persistent storage troubleshooting.

This removes empty documentation pages and places the
troubleshooting docs to the top of the docs/source tree. Also
places the pod disruption budget docs to the rest of the
getting-started docs, which are primarily concepts used in
openstack-helm
